Personal information, payment information and other data transmitted through our system are encrypted using 128-bit SSL encryption technology. Security procedures are tested and reviewed on a regular basis.
Membership Waiver
I am an adult over 18 years of age and wish to participate in YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights activities. IN CONSIDERATION of being permitted to utilize the facilities, services and programs of the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights for any purpose, including, but not limited to observation or use of facilities or equipment, or participation in any off-site program affiliated with the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights, the undersigned, for himself or herself and any personal representatives, heirs, and next of kin, hereby acknowledges, agrees and represents that he or she has, or immediately upon entering or participating inspected and carefully considered such premises and facilities or the affiliated program. In addition, I give my children permission to participate in YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights activities. I understand that even when every reasonable precaution is taken, accidents can sometimes happen. Therefore, in exchange for allowing me to participate in YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights activities, I understand and expressly acknowledge that I, for myself, or anyone entitled to act on my behalf, waive and release the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights, sponsors, representatives, and successors from all claims or liabilities of any kind arising out of my participation in activities at or sponsored by the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights. I further agree to indemnify and save harmless the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights from any claims or demands arising out of any such injuries or losses. I understand that this release includes any claims based on negligence, action or inaction of the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights, its staff, directors, members and guests. I have read, understand, and am voluntarily signing this authorization and release.
I understand that the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights is not responsible for personal property lost, damaged, or stolen while members and / or program participants are using YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights, on YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights premises, or involved in YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights programs.
I give my permission to the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights to use limitation and obligation, photographs, film footage, or tape recordings which may include my image or voice for the purposes of promotion or interpreting YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights programs.
Membership Agreement
YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights membership is a seasonal membership plan. I understand that this membership will remain in effect for as long as I retain the membership card issued to me. I further understand that membership dues are nonrefundable. Membership cards are the property of the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights and must be surrendered upon demand. All membership records, files, notes, paper and data are the property of the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights.
All membership rates are subject to change with 30 days written notice. I understand it is my responsibility to notify the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights of any change in address, bank account information (if utilizing bank draft for payment of dues) or credit card information/expiration date (if utilizing credit card for payment of dues).
I authorize the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights to send occasional emails about programs, news and discounts (we promise not to abuse that privilege). I understand that the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights will keep my email address private and will never sell/give out my email address.
I understand that the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights has the sole authority to terminate any season pass with or without notice to ensure the safety and comfort of the general membership. The YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights reserves the right to review any application and request additional documentation, at any time, to ensure the accuracy of information and compliance to YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights membership standards and policies. The YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights also has the right to request additional documentation. The YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights further reserves the right to review any membership application and deny membership to any member who may pose a potential threat to the security and/or safety of other members.
I will receive the Kroger Aquatic Center Guide that communicates all benefits of a Season Pass. I further understand that YMCA management has the authority to amend policies and procedures at any time to ensure the delivery of the highest possible services to the general membership. I acknowledge the waiver and membership agreement set forth above, and understanding the policies of the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights, hereby apply for membership. Note: Parent or guardian must sign if applicant is under 18 years of age.
Electronic Funds (EFT) or Credit Card Authorization
I authorize my bank to honor preauthorized Electronic Funds Transfer (or credit card institution) drawn by the YMCA of Greater Dayton/Kroger Aquatic Center/City of Huber Heights on my account for (membership/program) payments as indicated below. When the bank honors the EFT (or credit card) by charging my account, such transfer shall constitute notice of payment due and my receipt for the payment. Should any preauthorized EFT (or credit card) not be honored by said bank when received by them, then it is understood that the payment is to be made by me in the amount of said payment plus posted returned draft/check fee. It is further understood that if such payment is not honored by the bank (or credit card institution), then the YMCA, at its discretion, may resubmit the amount due for payment on a future date.
Additionally, I authorize the YMCA to draw via EFT the additional fees associated with season pass for additional children added to the membership as defined below. I understand that this will be charged at a rate of $10 per child for children residing in the household.
Family Membership Definition
Definition of a Family Season Pass to the Kroger Aquatic Center at The Heights: Family Season pass include 4 family members with each additional youth family member costing $10.00 each. A Family is defined as either; two adults in a household, and two children ages 18 or under. Or single adult, and up to 3 children residing in the same household or claimed on their most recent federal income tax return (proof may be required at time of purchase). Neighbors, visitors, baby sitters, etc. are not included.
